A person can access a grant where he\/she:<\/p>\n

    \n
  • is a citizen of Trinidad & Tobago and is ordinarily resident in Trinidad & Tobago<\/li>\n
  • is unmarried and under the age of eighteen years at the time of the application<\/li>\n
  • has been referred by a medical specialist in the particular field of treatment required<\/li>\n
  • requires treatment that is unavailable at a local medical institution<\/li>\n
  • requires treatment for a life-limiting illnesses covered under the Act<\/li>\n
  • has medical expenses that exceed twenty-five percent of the income of his\/her family.<\/li>\n<\/ul><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

    \n\t\t\t\t\t\t\t\t\t\t\t\t\t\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/path><\/svg><\/span>\n\t\t\t\t\t\t\t\t<\/path><\/svg><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\tWhat is the Application and Assessment process?<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t

    Application<\/strong>

    1. Collect the form from the CLFA office at Building 16, Wendy Fitzwilliam Paediatric Hospital, Eric Williams Medical Complex, Champs Fleurs, or download it from the CLFA\u2019s website.<\/li>
    2. Submit the signed and completed form and all relevant documents (these forms must be submitted in person).<\/li>
    3. Your application is only deemed complete when all original documents have been verified in person.<\/li><\/ol><\/p>\n

      Assessment<\/strong>

      1. The Life Unit coordinates a clinical and financial assessment, and a recommendation is submitted, through the Family Case Manager and Chief Executive Officer, to the Board of Management.<\/li><\/ol><\/p>\n


        If the application is approved:<\/b>
        <\/p>\n

          \n
        1. The CLFA assists the family with travel arrangements, including communication with the receiving hospital, travel bookings and communications with Immigration and Embassy authorities (if required). <\/li>
        2. All payments are made directly to the service providers.<\/li>\n
        3. The progress of the patient’s care abroad is monitored with support being provided by the assigned CLFA Case Officer.<\/li>\n
        4. Upon return to T&T, the parent\/legal guardian meets with a representative from CLFA to provide relevant feedback.<\/li>\n<\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t
          \n\t\t\t\t\t
          \n\t\t\t\t\t\t\t\t\t\t\t\t\t\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/path><\/svg><\/span>\n\t\t\t\t\t\t\t\t<\/path><\/svg><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\tHaving received a grant under the Act, can I apply for another grant?<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t

          A person cannot receive more than one (1) grant for the same medical condition within a two-year period unless there is an exceptional circumstance.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

          The grant facilitates only one (1) accompanying adult (parent, legal guardian, or medical social worker) unless under special clinical or social circumstances as approved by the CLFA.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

          Yes, hotel\/flights\/accommodations are paid for the travelling parent and the child, as long as these costs are within the $1M limit stipulated in the CLF Act.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t
