CAN I APPLY?<\/span><\/strong><\/p>

A person can access a grant where he\/she meets all the following criteria:<\/span><\/p>

1.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Is a citizen of Trinidad & Tobago and is ordinarily resident in Trinidad & Tobago.<\/span><\/p>

2.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Is unmarried and under the age of eighteen years at the time of the application.<\/span><\/p>

3.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Has been referred by a medical specialist in the field of treatment required.<\/span><\/p>

4.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Requires treatment that is unavailable at a local medical institution.<\/span><\/p>

5.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Requires a lifesaving procedure.<\/span><\/p>

6.\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Has medical expenses that exceed twenty-five percent of the income of his\/her family.<\/span><\/p>

APPLYING\u00a0<\/b>FOR FUNDING<\/b><\/p>

Collect the application form from the CLFA office at the Wendy Fitzwilliam Paediatric Hospital, Eric Williams Medical Complex, Trinidad & Tobago or download it from our website www.childrenslifefund.org.tt.<\/span><\/p>

Submit the signed and completed form and all relevant documents.<\/span><\/p>

The Life Unit coordinates a clinical and financial assessment, and a recommendation is submitted, through the Chief Executive Officer to the Board for review.<\/span><\/p>

Once a decision is made you will be informed. <\/span><\/p>

If the application is approved the CLFA makes the relevant travel arrangements which include:<\/span><\/p>

\u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Applying for the VISA for the child
and 1 parent\/guardian,<\/span><\/p>

\u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Booking the necessary flights<\/span><\/p>

\u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>Booking the accommodation<\/span><\/p>

The CLFA makes the payment arrangements for treatment directly to the medical
institution.<\/span><\/p>

The patient is monitored and evaluated during treatment while abroad.<\/span><\/p>

Upon return the parent\/legal guardian meets with a representative from CLFA to provide feedback.<\/span><\/p>

IMPORTANT\u00a0<\/span>TO NOTE<\/span><\/p>

\u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>A person cannot receive more than one grant for the same medical condition within a two-year period unless there is an exceptional circumstance.<\/span><\/p>

\u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <\/span><\/span>The maximum value of the grant shall not exceed one million Trinidad and Tobago dollars.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t
