CAN I APPLY?<\/span><\/strong><\/h2>

A person can apply for a grant where he\/she meets the following criteria:<\/p>

\u2022 a citizen of Trinidad and Tobago and is ordinarily resident in Trinidad and Tobago,
\u2022 unmarried and under the age of eighteen (18) years at the time of the application,
\u2022 referred by a medical specialist in the field of treatment required,
\u2022 in need of treatment that is unavailable at a local medical institution,
\u2022 diagnosed with a life-limiting illness, and
\u2022 has medical expenses that exceed twenty-five (25%) percent of the income of his\/her family.<\/p>

APPLYING\u00a0<\/b>FOR FUNDING<\/b><\/p>

Collect the application form from the CLFA office at the Wendy Fitzwilliam Paediatric Hospital, Eric Williams Medical Complex, Trinidad & Tobago or download it from our website www.childrenslifefund.org.tt.<\/p>

Submit the signed and completed form and all relevant documents.<\/p>

The Life Unit coordinates a clinical and financial assessment, and a recommendation is submitted, through the Chief Executive Officer to the Board for review.<\/p>

Once a decision is made you will be informed.<\/p>

If the application is approved the CLFA makes the relevant travel arrangements which include:<\/p>

  1. Applying for the VISA for the child and 1 parent\/guardian,<\/li>
  2. Booking the necessary flights<\/li>
  3. Booking the accommodation<\/li><\/ol>

    The CLFA makes the payment arrangements for treatment directly to the medical\u00a0institution.<\/p>

    The patient is monitored and evaluated during treatment while abroad.<\/p>

    Upon return the parent\/legal guardian meets with a representative from CLFA to provide feedback.<\/p>

    IMPORTANT\u00a0TO NOTE<\/strong><\/p>

    \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0 A person cannot receive more than one grant for the same medical condition within a two-year period unless there is an exceptional circumstance.<\/p>

    \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0The maximum value of the grant shall not exceed one million Trinidad and Tobago dollars. The Minister of Health can approve up to 1.5 million in exceptional cases and can adjust limits by\u00a0Order.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t
